• Conduct telephonic outreach to patients and/or family members to assess for disease exacerbations
• Collect and document comprehensive clinical history
• Assess patients for exacerbations of chronic conditions including CHF, COPD, diabetes, dehydration, and others as needed
• Interpret electronic patient dashboards to support clinical decision-making
• Notify the patient's PCP and relevant third partiesin the event of an exacerbation
• Document all assessments electronically ina timelyandaccuratemanner
• Submit timesheets on time through theCare Linxapp
• Communicate with your Care Advisorregardingany case updates or changes in patient condition
• Maintain client confidentiality at all times

Required Qualifications:

Active RN license in California and Utah; licensure in additional states preferred

Bilingual fluency in English and Spanish required

Minimum 2 years of clinical experience in one or more of the following:
Acute Care ED, ICU, Telemetry, Stepdown/Progressive Care Unit

Experience conducting clinical history and assessments with older or geriatric patients

Access to a smartphone capable of running theCareLinxapp (required for scheduling, documentation, and timesheets)

Must pass a background check
