** Join our compassionate and collaborative team as a Community Behavioral Health Liaison, where you will play a vital role in connecting individuals to critical behavioral health and community-based services. This position offers the opportunity to make a meaningful impact by supporting individuals in accessing care, navigating systems, and achieving stability and improved wellbeing. If you are passionate about advocacy, relationship-building, and helping others overcome barriers, this role allows you to contribute to a mission-driven organization focused on recovery, resiliency, and community this role, you will act as a liaison between clients, providers, and community resources to ensure coordinated, person-centered care.

* Key Responsibilities:
*
* · Serve as a liaison between clients, providers, and community resources to ensure coordinated care
· Engage individuals and assess needs related to behavioral health, social services, and community support
· Facilitate referrals and connect clients to appropriate services including medical, mental health, housing, and social supports
· Collaborate with interdisciplinary teams to develop and support individualized service plans
· Advocate on behalf of clients to reduce barriers to care and improve access to services
· Build and maintain relationships with community partners and service providers
· Monitor client progress and follow up to ensure continuity of care
· Maintain accurate and timely documentation in accordance with organizational and regulatory standards
· Participate in team meetings, case reviews, and program development activities
· Support outreach efforts and community engagement initiatives
· Promote a recovery‐focused, trauma‐informed, and culturally responsive approach to services
· Perform other duties as assigned by leadership
** Education, Experience, and/or Credential
Qualifications:
*
* · Bachelor’s degree in social work, psychology, counseling, or a related human services field preferred
· Relevant experience in behavioral health, case management, or community services may be considered in lieu of education
· Knowledge of community resources and behavioral health systems preferred
*

* · Must have a valid driver’s license with an acceptable driving record and current auto insurance
· Strong communication and relationship‐building skills
· Ability to work effectively with diverse populations and maintain confidentiality
· Strong organizational and time‐management skills
· Ability to multi‐task and respond effectively in a fast‐paced environment
· Ability to collaborate with interdisciplinary teams and community partners
** Physical Requirements*
* · Ability to exert up to 10 pounds of force occasionally
· Ability to perform repetitive hand, finger, and arm movements for typing and documentation
· Ability to sit for extended periods with occasional walking or standing
