The Patient Navigator manages a variety of front office functions and is key to the smooth operation of a dynamic outpatient wound care center. The role performs general office duties to assist the staff of the WCC and supports patients throughout the care continuum.
Responsibilities- Coordinate with Center leadership to maximize daily patient census.
- Actively participate in staff meetings to support key functions.
- Greet patients and visitors, answer and route calls to appropriate staff.
- Maintain all patient communication needs including scheduling, rescheduling, and appointment reminders.
- Work and schedule for provider, according to the care continuum model.
- Obtain and verify patient insurance information, including pre-certifications and pre-authorizations, and enter data in appropriate databases.
- Collect and enter patient charges, verify and reconcile charges, and prepare monthly patient survey data for providers.
- Assemble new patient charts, maintain and file existing charts, and conduct spot checks for data completeness and signatures.
- Coordinate and schedule ancillary testing with other hospital departments.
- Arrange patient transportation as needed.
- Maintain office equipment and supplies, and medical supplies as directed.
- Perform other duties as required.
Education and Experience
- High School Diploma or GED;
Associate’s degree preferred. - Minimum of two (2) years of office administration experience, preferably in a medical setting.
- Prior medical coding experience preferred.
- Proficient in Microsoft Office (Word, Excel, Outlook).
- Good customer, interpersonal, and communication skills (oral and written).
- Strong organization and time‑management skills.
- Typing speed of 60 words per minute.
- Basic math skills and attention to detail.
- Ability to maintain confidentiality.
- Capacity to work in a fast‑paced environment and manage multiple projects simultaneously.
- Team‑orientated and collaborative work style.
