Job Purpose:

* The Targeted Case Manager I at LIFESTREAM BEHAVIORAL CENTER is responsible for providing comprehensive case management services to individuals with mental health and behavioral challenges. The role involves assessing client needs, developing individualized care plans, and coordinating services to ensure clients receive the necessary support for their well-being and recovery.

Key Responsibilities:

* Conduct thorough assessments of clients' needs and strengths to develop personalized care plans.

* Coordinate and facilitate access to necessary services, including medical, educational, and social support.

* Monitor and evaluate client progress, adjusting care plans as needed to ensure effective support.

* Advocate for clients' needs and rights, ensuring they receive appropriate resources and services.

* Maintain accurate and up-to-date client records and documentation in compliance with organizational and legal standards.

* Collaborate with a multidisciplinary team of professionals to provide holistic care and support to clients.

* Provide crisis intervention and support to clients and their families as needed.

* Educate clients and their families about available resources and support systems.

* Participate in ongoing training and professional development to stay current with best practices in case management.

* Adhere to all organizational policies and procedures, ensuring ethical and professional conduct at all times.
