Behavioral Case Manager II – Licensed/Certified Behavioral Health Role

Locations: Candidate must reside in the state of Virginia.

Field:
This role enables associates to primarily operate in the field, traveling to client sites or designated locations as required, with occasional office attendance for meetings or training. This approach ensures flexibility, responsiveness to client needs, and direct, hands‑on engagement. Candidates not within a reasonable commuting distance from the posting location(s) will not be considered for employment, unless an accommodation is granted as required by law.

The Behavioral Case Manager II is responsible for performing case management telephonically and/or by home visits within the scope of licensure for members with behavioral health and substance abuse or substance abuse disorder needs.

How You Will Make an Impact
  • Provide case management services to individuals with serious mental illness (SMI), including those with co‑occurring substance‑use disorders and/or significant physical health conditions.
  • Deliver services primarily through telephonic outreach, with the possibility of in‑person visits when clinically necessary.
  • Conduct regular outreach to assigned members to build rapport, assess needs, link to needed services/resources and provide short‑term support (up to 6 months).
  • Support members by helping them navigate urgent and non‑urgent needs and connecting them to appropriate resources.
  • Respond to more complex cases and account‑specific requests.
  • Use appropriate screening criteria knowledge and clinical judgment to assess member needs.
  • Conduct assessments to identify individual needs and develop a specific care plan addressing objectives and goals as identified during assessment.
  • Monitor and evaluate effectiveness of care plan and modify the plan as needed.
  • Help members access appropriate, high‑quality and cost‑effective care.
  • Coordinate with internal and external resources to meet identified needs of the members and collaborate with providers.
  • Serve as a resource to other BH Case Managers.
  • Participate in cross‑functional team projects and initiatives.
Minimum Requirements
  • Requires a MA/MS in social work counseling or related behavioral health field, or a degree in nursing, with a minimum of 3 years of clinical experience in social work counseling addressing complex psychiatric and substance‑abuse or substance‑abuse disorder treatment; or any combination of education and experience providing an equivalent background.
  • Current active unrestricted license such as RN, LCSW, LMHC, LICSW, LPC (as allowed by applicable state laws), LMFT, LMSW (as allowed by applicable state laws) or clinical psychologist to practice within the scope of licensure in applicable states or territories.
  • Previous experience in case management and telephonic and/or in‑person coaching with members with complex psychiatric/substance‑abuse and/or medical disorders.
Preferred Skills, Capabilities and Experiences
  • Experience in health coaching and motivational interviewing techniques.
  • Knowledge of behavioral health systems of care in Virginia.
  • Strong written and verbal documentation skills.
  • Intermediate to advanced Microsoft Office usage.
