Nurse Practitioner Liaison

Minimum of five (5) years clinical experience at an acute care of ambulatory setting required.

Roles & Responsibilities

Liaison – Communicate lab and diagnostic test results to patients per provider instructions. Lab Beans – Should review about 50 labs for providers. Lab Review – report out on all normal labs and communicate with patients. Serve as a point of contact for patient phone inquiries, addressing concerns and escalating as needed. Referrals Address the referrals that have received care and follow up on any pending referrals by reaching out to the patients.

Process chronic prescription refills in accordance with provider directives and organizational policies. Support providers with clinical tasks, ensuring efficient workflow and patient-centered care. Collaborate with the clinical access team to facilitate timely patient access to care. Triaging the documents from outside visits to determine any issues that need to be addressed in a timely manner. Assist in completion of documents including conducting patient visits for form completion if necessary.

Conduct specific types of patient visits. Maintain accurate and timely documentation in the electronic health record (EHR). Adhere to all clinical protocols, regulatory requirements, and best practices. Participate in quality improvement initiatives to enhance patient outcomes.

Nurse Practitioner

Promotes the mission, vision, and values of Health Works. As part of the Health Works team, assists in both the planning and implementation of Healthworks' health care plan for patients. Assumes quality of care for all patients. Provides appropriate, efficient, and effective clinical medical care required in the prevention, diagnosis, and treatment of human disease and injury. Evaluates situations, which call for emergency medical procedures or require physician intervention.

Responsible for diagnosis and treatment of conditions within the scope of practice. Reviews and updates the health record using EMR. Evaluates patients including obtaining medical history and physical exam. Orders and interprets appropriate lab and x-ray data. Within standard care guidelines, diagnoses patient conditions, prescribes medications, orders tests, discriminates between normal and abnormal finding and provides treatment and therapy. Initiates appropriate referrals to other health care providers.

Seeks supervision from Center physicians when care or conditions are beyond scope of practice or knowledge base. Obtains appropriate consults from specialists when needed. Clearly documents all patient visits and billing as soon as possible or within 24 hours following that encounter. All entries in the patient medical record are to be entered into the EMR using the SOAP format.

Provides high quality customer service to patients with a high emphasis on customer satisfaction reports. Shows effort to maintain patient flow and efficiency which directly impacts patient and staff satisfaction. Practices fiscally responsible medicine to help sustain the financial stability of the health center. This includes using laboratory formulary, a stepwise approach to diagnostic testing, and using generic medications when medically appropriate, etc.

Works with facility staff and medical staff through the processes of quality improvement, peer evaluation, professional and staff meetings, and federal and state rules and regulations to ensure quality clinical standards are maintained at the facility.
