Overview
The Clinical Mental Health Professional utilizes clinical expertise to perform psychosocial assessments, develop and implement plans of care appropriate to the care team, assess crisis situations to provide needed resources or referrals, and provide family and individual treatment as indicated. This position works collaboratively with patients, families, healthcare providers, insurers, community resources, and all other involved parties. This role provides care aligned with the established plan of care in the assigned patient population.

- Pay Range Certified: $32.02-$49.44 Non Exempt

- Diagnoses and treats various mental, emotional, and behavioral disorders, modifying care plans to reflect changes in patient status.
- Refers patients to other services to improve well-being, including community services, treatment programs, support groups, psychiatrists, or other mental health providers.
- Works with the multi-disciplinary team to develop a discharge plan to ensure continuity of care.
- Organizes and prioritizes daily work by assessing new, current, and discharging patient needs.
- Performs behavioral health consultation for patients and caregivers; conducts psychosocial assessments; provides supportive, crisis, bereavement, and other behavioral health interventions as necessary.
- Educates, communicates, and coordinates with multiple patient care teams and outside agencies to ensure quality care, timely discharge, and to identify and resolve delays; credentialing through medical staff office and peer review requirements may apply in behavioral health settings.
- May provide support and debriefing for staff following adverse events in conjunction with the Employee Assistance Program.
- Master's Degree in Social Work or related field from an accredited institution. Degree will be verified.
- Current State Licensure as CSW, LCSW, LMFT or CMHC.
- Excellent written and verbal communication skills.
- Computer Skills, e.g., email, word processing.
- Experience with patients, families, and treatment teams.
- Ongoing need to see and read information, assess patient needs, operate monitors, and identify equipment and supplies.
- Frequent verbal communication with patient care teams, patients, and visitors; ability to hear and understand alarms and information quickly, especially during emergencies.
- Manual dexterity to manipulate equipment; frequent computer use for documenting patient care and medication preparation.
- Ability to lift and transfer patients; ability to bend, lift, and carry supplies and equipment of varying weights.
- Ability to walk, assist with transporting/ambulating patients, and navigate busy environments; may involve standing for extended periods.
