Position Summary:

Revere Health is seeking a dedicated and compassionate Advanced Practice Practitioner (APP)—either a Nurse Practitioner (NP) or Physician Assistant (PA)—to join our dynamic Cardiology team. This role supports our mission to deliver high-quality, value-based care to patients with cardiovascular conditions.

Essential Job Functions:

• Conduct comprehensive patient assessments, including history, physical exams, and diagnostic evaluations.
• Collaborate with cardiologists to develop and implement individualized treatment plans.
• Manage chronic cardiovascular conditions and provide follow-up care.
• Educate patients and families on disease prevention, treatment plans, and lifestyle modifications.
• Participate in quality improvement initiatives and interdisciplinary team meetings.
• Document patient encounters accurately and timely in the EMR system.

Qualifications:

• Current licensure as a Nurse Practitioner or Physician Assistant in the state of Utah.
• Certification in cardiology or relevant clinical experience preferred.
• Strong clinical assessment and diagnostic skills.
• Excellent communication and teamwork abilities.
• Commitment to patient-centered, value-based care.

Hours:

Monday - Friday 8 - 5
