* Job Description:

* + Perform all pharmacist functions including compounding, dispensing, verification, and documentation of medications across central, satellite, and IV areas.

+ Review and monitor medication orders and patient profiles to prevent, identify, and resolve drug-related problems; complete pharmacist consults and perform pharmacokinetic dosing, monitoring, and adjustments.

+ Provide safe, timely, and cost-effective medication therapy; actively participate in stewardship, dose optimization, surveillance, and restricted-use programs.

+ Engage in direct patient care and interdisciplinary rounds, ensuring smooth transitions of care from admission through discharge and follow-up.

+ Educate and counsel patients, families, and healthcare team members on safe and effective medication use.

** Knowledge, Skills, and Abilities:*
* - Proficiency in utilizing EPIC electronic health records (EHR) and clinical decision support systems

- Perform Clinical Consultations on Glycemic Management, Anticoagulation, and Pharmacokinetics

- Must possess the ability to establish and maintain effective relationships with hospital staff and physicians as well as the public

- American Society of Health-System Pharmacist (ASHP) Board Certification [Preferred]

- PGY1 Pharmacy Residency [Preferred]
