- Explains hospice services and Medicare benefits to patients and families
- Submits documentation and paperwork to facilities at the completion of patient visits
- Participates as a member of the IDT, including development of the plan of care

- Completes psycho-social assessments for patients
- Master of Social Work (MSW) from a CSWE accredited program
- At least 1 year of healthcare experience
- State licensure as required for the state of practice
- Ability to read, analyze, and interpret general business documents
