Speech Language Pathologist

Seeking a licensed Speech Language Pathologist to provide personalized, in-home and community-based care to older adults, promoting mobility, independence, and quality of life.

• Conducting evaluations, developing individualized treatment plans, and delivering evidence-based therapy.
• Supporting patient wellness, functional independence, and mobility through therapy sessions.
• Documenting care accurately using electronic medical records in compliance with regulations.
• Collaborating with interdisciplinary teams including PTs, OTs, and caregivers.
• Educating patients, families, and staff to enhance safety and aging-in-place outcomes.
• Utilizing telehealth and virtual care options to increase flexibility and access.

• Valid state licensure or eligibility, with a degree from an accredited Speech Language Pathology program.
• Proficiency in basic computer skills and EMR systems.
• Current CPR certification and ASHA CCC.
• Ability to meet physical demands such as lifting, repositioning patients, and driving between locations.
