Bronson at Home is seeking a dedicated and compassionate Physical Therapist to join our growing Home Care team. In this rewarding role, you'll help patients regain mobility, restore function, manage pain, and improve their overall quality of life through personalized, one-on-one care delivered in the comfort of their homes.

What You’ll Do
  • Evaluate patients' physical abilities, limitations, and rehabilitation needs through comprehensive assessments
  • Develop individualized treatment plans designed to improve mobility, strength, balance, coordination, and function
  • Provide skilled physical therapy interventions in the home setting
  • Establish realistic goals and measure patient progress throughout the course of care
  • Educate patients, families, and caregivers on exercises, safety techniques, and self-management strategies
  • Collaborate with physicians, nurses, therapists, and other healthcare professionals to coordinate care
  • Participate in interdisciplinary care planning and team discussions
  • Recommend and train patients on assistive devices, adaptive equipment, and mobility strategies as appropriate
  • Support quality improvement initiatives and program development efforts
  • Serve as a clinical resource and mentor within the rehabilitation team
Qualifications Required
  • Graduate of an accredited Physical Therapy program
  • Current Physical Therapist license in good standing with the State of Michigan
  • Current Michigan driver's license
  • Reliable transportation available for daily business use
  • Current automobile insurance
  • BLS certification within three months of hire
  • Strong communication and interpersonal skills
  • Basic computer and mobile device proficiency
Preferred
  • Previous clinical experience in physical therapy
  • Home care experience

Key Responsibilities Clinical Care
  • Interpret and implement physician referrals
  • Evaluate range of motion, strength, coordination, balance, endurance, and functional abilities
  • Establish functional baselines and monitor patient progress throughout treatment
  • Deliver evidence-based physical therapy interventions
  • Facilitate patient participation in therapeutic activities and exercise programs
  • Provide patient, family, and community education
  • Assist with goal setting and determination of treatment duration
  • Design or recommend assistive devices and therapeutic supports as appropriate
  • Provide coverage and support across Rehabilitation Services areas when needed
Documentation & Administration
  • Complete accurate and timely documentation of evaluations, treatment plans, interventions, and patient progress
  • Maintain compliance with electronic health record documentation requirements
