Public Health, Environment, and Health Care Finance (which includes the State’s Medicaid program, Kan Care). The position (K0247308)

Provide ongoing technical assistance to CHWs implementing Food is Medicine (FIM) activities, including food insecurity, screening, referral processes, nutrition education, and participant follow-up. This role acts as a resource for CHWs regarding FIM models, chronic disease prevention, and health-related social needs, while addressing daily implementation challenges. Through individual and group coaching, strengthen CHW skills, confidence, and role clarity across diverse clinical and community settings.

Develops and delivers initial and ongoing training for CHWs focused on FIM workflows, documentation requirements, professional boundaries, and best practices. This includes supporting onboarding of new CHWs and adapting training content to meet community needs. Working with clinical partners, community-based organizations, and program leadership to support effective integration of CHWs into care teams. Including clarifying CHW roles, communication pathways, referral processes, and bridging community and clinical systems to improve coordination and effectiveness.

Develops education and supports the engagement of Community Health Workers (CHWs), healthcare providers, the general public, and other relevant stakeholders in Kansas. Collaborates with partners at the local, state, and national levels to align community health worker initiatives with Food is Medicine and chronic disease programs, promoting them both locally and statewide.
