Home Health - Full-Time/PRN - Per Visit - Registered Nurse. South Dallas, 75215. $2,500 Sign-on Bonus (For Full Time Only).

Description:

Our full-time Registered Nurse (RN) will have a variety of duties that support continuity of care. The candidate must maintain 32 productivity points weekly to qualify and must participate in the compensated on-call program. This RN will see visits assigned within the scope of practice of their license, including OASIS visits, skilled nursing visits, and supervised visits. They will also participate in the compensated perception of new hires.

Administer skilled nursing care for patients of all ages in their place of residence and coordinate care with the interdisciplinary team, patients, families, and referring agency. Assumes responsibility for, and participates in, the coordination of care.

• Case management and supervised visits.
• Perform skilled and comprehensive assessment of the patient during the initial admission.
• Assess and chart observations of the patient’s condition at each visit (vital signs).
• Provide skilled nursing care as needed in accordance with the plan of care (administering medication, cleaning/dressing wounds, assessing healing progress).
• Provide proper patient education for home health care procedures and strategies.
• Maintain clinical records: visit notes (completed within 24 hours), comprehensive assessments including OASIS (completed within 48 hours), medication profile, and orders/plan submitted in a timely manner.
• Maintain confidentiality of patient PHI in accordance with HIPAA.
• Support patient comfort through palliative care.

• Current Texas State license as a Registered Nurse.
• 2+ years of experience as an RN in Home Health.
• Current CPR certification.
• Reliable transportation.
• Texas Driver’s License and Auto Liability Insurance.
• Current TB Skin Test.
• EMR experience (Kinnser preferred).
