Job Title: Home Health Weekend RN – Registered Nurse

Location: Lenoir City, TN

Job Type: Part Time

Work Location: On the road (multiple patient residences within the assigned geographic area)

The RN provides and directs provisions of nursing care to patients in their homes as prescribed by the physician and in compliance with applicable laws, regulations, and agency policies. The role includes coordinating the total plan of care with other health care professionals, conducting initial and comprehensive nursing evaluations, developing plans within state guidelines, ensuring eligibility and medical necessity, maintaining accurate documentation, and delivering high‑quality clinical services from admission through discharge.

• Coordinate the plan of care with physicians, agency personnel, patients, families, and community resources to maintain continuity of care.
• Conduct initial and/or comprehensive nursing evaluation visits, determine primary focus of care, and develop the plan of care within state‑specific guidelines.
• Ensure patients meet home health eligibility and medical necessity guidelines as defined by the payer.
• Maintain and submit accurate documentation and clinical notes, including patient condition, goals, interventions, and responses to care.
• Provide high‑quality clinical services within the scope of practice as defined by state laws governing nursing practice.

• One (1) year of clinical RN experience.
• Current RN license in the state where services are rendered.
• Current CPR certification.
• Valid driver’s license, vehicle insurance, and dependable transportation (own vehicle or public transport).
• Reliable commute to multiple patient residences daily within the assigned geographic area.
