UMass Memorial Health in Leominster is looking for a Diabetes Educator to oversee the implementation of Diabetes Self-Management Education and Support (DSMES) plans. The role involves collaborating with healthcare providers, conducting assessments, and designing individualized education plans for patients with diabetes. A Bachelor’s degree in Food and Nutrition or a related field and certification as a Diabetes Educator within two years are required.
