Position:
Licensed Practical Nurse – Home Care (Cost Center: 24100 – 4320)

Exemption Status: Non‑Exempt

Hiring Range: $23.28 - $41.90 per hour (final offer may vary within this range based on experience, skills, qualifications, and internal equity considerations)

Schedule Details: Monday through Friday, Holiday – one major summer holiday, Holiday – one major winter holiday, Weekends – every fourth weekend

Shift: Day Shift, 8 Hours (United States of America)

Hours: 40 per week

Sign‑On Bonus: $7,500.00

• Provide patient care according to standards of practice and departmental policy and procedure.
• Assist in patient education and document appropriately per policy and procedure.
• Achieve continuity of patient care through planning and exchange of information with other health care professionals.
• Administer medications safely and in accordance with professional standards of practice and departmental policies and procedures within the scope of practice for the LPN.
• Make recommendations for changes to the plan of care to the appropriate RN or other team member as the patient condition warrants.
• Delegate patient care activities to appropriate personnel in accordance with professional standards of practice and departmental policy and procedure and within scope of practice.
• Meet productivity guidelines (7 visits per 8‑hour day) according to departmental policy and procedures.
• Support performance improvement initiatives through active participation in PI processes, projects, and teams.
• Create and maintain documentation that is consistently complete, accurate, timely and legible in accordance with departmental policies and procedures.
• Maintain a safe environment for patients through consistent compliance with policies and procedures.
• Possess and apply the skills and knowledge necessary to provide care to patients and customers throughout the life span, with consideration of aging processes, human development stages and cultural patterns in each step of the care process.

• License/Certification/Education – Required
  • Current licensure as a Licensed Practical Nurse in the Commonwealth of Massachusetts.
  • Successful completion of an accredited Licensed Practical Nurse Program.
  • Valid U.S.
    
    -issued driver’s license.
  • Registered, inspected, and insured automobile for work‑related purposes.
• License/Certification/Education – Preferred
  • Current Basic Life Support (BLS) certification.
  • Current CPR certification.
• Experience/Skills – Required
  • Ability to perform detailed assignments with minimal supervision and work under pressure.
  • Knowledge of the nursing process for the delivery and management of patient care.
  • Ability to create electronic documentation that is accurate, intelligible, and concise.
  • Ability to complete and maintain all required licensed practical nurse competencies.
  • Ability to communicate proactively and clearly with patients, caregivers, and medical staff.
  • Ability to read, write and speak clearly in English.
• Experience/Skills – Preferred
  • Minimum two (2) years of medical‑surgical nursing experience in direct patient care.
• Unless certification, licensure or registration is required, an equivalent combination of education and experience that provides proficiency in the areas of responsibility listed in this description may be substituted for the above requirements.
