Position Overview

The Residential Case Manager supports the Emergency Housing Assistance program by providing case management, service coordination, documentation, and stabilization support for Veterans participating in the residential program.

This role works directly with Veterans in a residential setting to identify needs, address barriers to housing stability, connect participants with appropriate services, and support progress toward individualized goals. The Residential Case Manager coordinates with program leadership, residential staff, VA resources, community providers, and internal programs to support resident stability, safety, and transition planning.

This position requires strong professional judgment, clear communication, trauma-informed engagement, and an understanding of the military and Veteran experience.

Key Responsibilities Residential Case Management and Veteran Support
  • Provide individualized case management to Veterans participating in the Emergency Housing Assistance residential program.
  • Assess resident needs, identify barriers, and develop service plans focused on housing stability, basic needs, wellness, and long-term stabilization.
  • Support Veterans in setting realistic goals and completing steps related to housing, benefits, healthcare, behavioral health, employment, transportation, and other support needs.
  • Maintain regular contact with assigned Veterans to monitor progress, provide guidance, and support follow-through on service plans.
  • Maintain appropriate professional boundaries while providing consistent, respectful, and recovery-oriented support.
Housing Stability and Service Coordination
  • Assist Veterans with housing-related needs, including housing search support, application preparation, documentation gathering, referrals, and transition planning.
  • Coordinate with VA programs, community housing providers, behavioral health providers, benefits agencies, and other external partners as appropriate.
  • Facilitate referrals and warm handoffs to internal programs and external service providers.
  • Work with Veterans to address practical barriers that may affect stability, including transportation, identification documents, income, benefits, healthcare access, and social support.
  • Support intake, stabilization, transition, and discharge planning in coordination with program leadership.
Safety, Crisis Awareness, and Supportive Engagement
  • Maintain awareness of resident needs, behavioral concerns, safety concerns, and potential signs of crisis or distress.
  • Identify potential crisis indicators and coordinate appropriate referrals, warm handoffs, or escalation to clinical, emergency, VA, or community resources as appropriate.
  • Communicate concerns through the proper chain of command and follow established procedures for incident reporting, safety concerns, and urgent situations.
  • Use trauma-informed, Veteran-centered communication when working with residents who may be experiencing stress, instability, mental health concerns, social isolation, or other barriers.
  • Maintain a calm, professional, and respectful presence when responding to challenging situations.
Documentation, Compliance, and Program Reporting
  • Maintain timely, accurate, and confidential case notes, assessments, referrals, service plans, progress updates, and related documentation.
  • Ensure documentation supports program requirements, grant expectations, audit readiness, and continuity of care.
  • Track resident progress, service connections, referrals, outcomes, and barriers as required.
  • Complete required reports, forms, and data entry accurately and within established timelines.
  • Maintain confidentiality of resident information in accordance with organizational policy and applicable privacy standards.
Communication, Coordination, and Program Support
  • Serve as a point of coordination between residents, program leadership, Residential Specialists, internal programs, and external service providers.
