Key Responsibilities:

• Provide emotional support and counseling to patients and their families to address their psychosocial needs
• Assess patients' mental health and social functioning to identify areas of need and develop individualized plans
• Collaborate with healthcare teams to ensure comprehensive care and optimal patient outcomes
• Facilitate patient education and resource referrals to promote healthy behaviors and disease management
• Maintain accurate and confidential records of patient interactions and outcomes
• Participate in interdisciplinary team meetings to share knowledge and best practices

Requirements:

• Experience in social work or a related field (recent graduates encouraged to apply)
• High school diploma or equivalent required; degree in social work or a related field preferred
• Strong communication and interpersonal skills
• Ability to work in a fast-paced environment with multiple priorities
