Position Overview

The Clinical Staff Pharmacist is responsible for ensuring safe, accurate, and effective medication therapy for patients in the acute care setting. This role collaborates with physicians, nurses, and the interdisciplinary healthcare team to optimize medication outcomes while maintaining compliance with regulatory and safety standards.

Key Responsibilities

Clinical Practice Reviews and verifies medication orders for accuracy, appropriateness, and safety Monitors drug therapy for effectiveness, interactions, and adverse reactions Provides pharmacokinetic dosing and therapeutic drug monitoring as needed Offers drug information and clinical consultation to providers and nursing staff Medication Management Oversees preparation and dispensing of medications Ensures compliance with regulatory, safety, and accreditation standards Monitors controlled substances according to policy Participates in medication reconciliation processes Collaboration & Patient Care Educates patients and healthcare staff regarding medication therapies Participates in interdisciplinary rounds as appropriate Promotes evidence-based medication practices Quality & Safety Supports quality improvement initiatives Ensures compliance with state and federal pharmacy regulations Maintains accurate documentation and reporting standards Requirements

Education Doctor of Pharmacy (Pharm

D) or Bachelor of Science in Pharmacy required Licensure & Certifications Active Pharmacist license in applicable state required BLS certification required Experience Minimum 1 year of pharmacist experience preferred New graduates considered Skills Strong clinical knowledge of pharmacotherapy Excellent communication and collaboration skills Ability to function in a fast-paced acute care environment Benefits
