* What You’ll Do:

*** Assess patient and family emotional, social, financial, and environmental needs (coping, safety, caregiver strain, resources, advance care planning readiness).
* Care coordination & resource navigation - connecting patients/families with community resources and coordinating with the interdisciplinary team.
* Participate in care plan development and case conferences.
* Serve as liaison between patients or families/caregivers and community agencies.

* What You Bring:

*** Current state licensure, or eligibility for licensure as required
* Strong skills in psychosocial assessment, counseling, and care coordination.
* Ability to support patients and families through serious illness, end-of-life care, and grief with compassion and professionalism
* Excellent communication, documentation, and time management skills.
