Key ResponsibilitiesPT Responsibilities
- Perform comprehensive physical therapy evaluations in patient homes.
- Develop individualized treatment plans focused on functional mobility, safety, and independence.
- Provide therapeutic interventions including gait training, strengthening, balance work, and home-exercise program development.
- Conduct home-safety assessments and recommend appropriate DME or environmental modifications.
- Manage a field-based caseload with strong organizational efficiency.
- Collaborate with interdisciplinary team members to ensure coordinated, patient-centered care.
- Maintain accurate, timely documentation in accordance with regulatory and organizational standards.
- Support safe transitions from hospitals, SNFs, and outpatient settings.
Full-Time Schedule
- Monday–Friday
- Daytime schedule
- Balanced caseload with strong team support
PT Requirements
- Current Idaho Physical Therapist license (or ability to obtain).
- One year of PT experience preferred; home-health experience a plus.
- Strong communication, assessment, and clinical reasoning skills.
- Ability to work independently in a field-based environment.
- Reliable transportation and valid driver's license.
Compensation Details
- Competitive salary: $105,000–$135,000
- $10,000 sign-on bonus
