Orchard View Post Acute Orchard View Post Acute is looking for a dedicated and detail-oriented MDS Coordinator to join our team. If you are an LPN or RN who enjoys combining clinical knowledge with coordination and care planning, this is an opportunity to grow your career in a supportive and collaborative environment.
Compensation $32 to $55 per hour, based on experience and qualifications
What You’ll DoAs our MDS Coordinator, you will play a key role in ensuring accurate assessments and high-quality care planning for our residents. You will collaborate closely with nursing, therapy, and interdisciplinary teams to support positive outcomes and regulatory compliance.
Coordinate and complete MDS assessments in accordance with federal and state regulations
Ensure accuracy and timeliness of documentation
Collaborate with the interdisciplinary team on care plans and resident needs
Monitor case mix index and identify opportunities for improvement
Stay current on MDS guidelines and regulatory updates
What We’re Looking For Active LPN or RN license in good standing
Experience with MDS preferred
Strong attention to detail and organizational skills
Ability to work independently while being a strong team partnerA positive, solutions-oriented mindset
