Job Overview

This position is part time, up to 20 hours a week with the potential to move into a full time role. Hours will be split between CBRS work and Case Management.

The Community Based Rehabilitation Specialist (CBRS) works in the home of clients and in the community to teach interventions and practice skills in order to support a client’s ability to manage functional needs independently. Workload is based upon patient needs and treatment/intervention activities designed to meet those needs.

The Case Manager ensures the smooth, cooperative functioning of the treatment program by serving as a member of the Treatment Team with the collaborative goal of integrating clinical and medication management services. The case manager will coordinate care by serving as a liaison between members of the multidisciplinary treatment team, including therapists, psychiatrist, consulting providers, referral source, family, and the client.

Brighter Futures Clinic offers specific therapeutic services to address both mental and behavioral health issues.

Brighter Futures clinic provides a variety of therapeutic services through clinically licensed counselors and/or social workers. Some therapeutic techniques used include individual, family and group psychotherapy. The clinic is trained to utilize Dialectical Behavioral Therapy (DBT) and Trauma Focused Cognitive Behavioral Therapy (TF-CBT) techniques.

Requirements
  • Must be at least 21 years of age.
  • Minimum 1 year of childcare experience.
  • Must have at least a Bachelor’s degree in Human Services OR Social Work field.
  • Valid state issued driver's license.
  • Able to pass background checks.
Preferred Qualifications

Paid experience in dealing with children/ youth in groups.

Work effectively with students, coworkers, community members and community agencies.

Trained in Trauma Informed Care (TIC), or able to attend TIC training immediately, and is actively engaged in furthering their TIC knowledge and training.
