Job Summary:

The Behavioral Health Integration Coordinator (Adult) will manage a dynamic case load and serve as a hub for the collaborative delivery of mental health, primary care as well as specialty care services within the Strong Futures Program. The Behavioral Health Integration Coordinator will be a link for patients transitioning from inpatient acute care, into a continuum of care services that support their on-going recovery goals and objectives.

They will ensure patients and families have access to an array of community resources as well a transitional plan within 6 months of their treatment initiation. Within the Behavioral Health Home Program, the coordinator will work closely with patients in the community, ensuring a delivery of care that addresses all aspects of physical and mental health.

Essential Duties and Responsibilities:
  • Supports and promotes the mission and values of Covenant Health Ministry.
  • Works with the inpatient team helping to identify patients needing referral to the Strong Futures services.
  • Attends treatment team meetings.
  • Helps to ensure a seamless transition of care from the inpatient care team to Strong Futures by helping arrange and /or scheduling appointments.
  • Supports and educates patients and families regarding transitional supports as well as community, helping remove access barriers to medical, dental, food-SNAP, childcare or other needs as identified.
  • Collaborates and communicates with patients’ community case manager while in the Strong Futures Program.
  • Assist patients/ families with Medicaid requirements and coverage renewals.
  • Contacts community resources routinely to establish availability and referral capacity.
  • Communicates professionally with referral sources and returns phone calls promptly.
  • Collect patient’s medical and behavioral history for presentation to the referral following HIPPA guidelines and system policies.
  • Attends patient appointments with medical and behavioral clinician as needed.
  • Works with the unit teacher, social worker and school systems to help patients with re-entry attending IEP/ 504 plans as needed.
  • When necessary, documents an individualized plan for each patient, listing goals and outcomes. May document on inpatients as well as patients receiving services in the Strong Futures program.
  • Reviews this treatment plan with the patient and family.
  • Documents progress towards the plan objectives
  • Documents contact with providers, patient and family.
  • Manages a dynamic case load ensuring that patients are actively participating in care.
  • Communicates regularly with clinical supervisor regarding individual patient progress or treatment issues.
  • Attends and participates in individual and team meetings with clinical supervisor. Informs clinical supervisor of any conflicts of interest, client crises, or other situations that require further review/ assessment.
  • Demonstrates ability to be open and willing to receive constructive feedback regarding client care.
  • Attends and actively engages in program meetings.
  • If needed helps follows up with families after a missed appointment.
  • Maintains all required competencies and completes annual compliance courses on time.
  • Supporting student learning is a shared responsibility. Every team member is expected to contribute to creating a welcoming environment and to actively assist students in their educational experience, regardless of their specific role.
  • Participates in community outreach activities.
  • Partners with unit staff/ and Strong Futures care providers, to do school and community marketing visits.
  • Other duties as consistent with this role.
