We are seeking an experienced Licensed Practical Nurse (LPN) to serve in a dual‑role position as Charge Nurse and Certified Medical Technician (CMT) for a 13‑week contract assignment in a long‑term care/skilled nursing setting. This role is ideal for an LPN who is confident in medication administration, leadership, and hands‑on resident care.

This position splits responsibilities between Charge Nurse duties and Medical Technician (CMT) functions. The LPN is responsible for medication administration for the assigned unit while also performing essential CNA‑related duties. The role reports directly to nursing leadership and follows established nursing standards and facility policies.

• Administer medications via oral, rectal, topical, inhalation, eye, and ear routes under RN supervision
• Maintain medication carts and ensure secure, organized storage areas
• Obtain authorization from licensed nursing staff prior to administering PRN medications
• Perform non‑narcotic medication counts at the beginning and end of shifts
• Monitor medication supplies and coordinate refills with nursing staff
• Take and document vital signs, including apical pulse rates
• Accurately document medication administration on the MAR
• Report significant changes in resident condition or vital signs promptly
• Provide direct resident care consistent with CNA responsibilities as needed
• Communicate effectively with nursing staff, medical providers, and interdisciplinary teams
• Demonstrate flexibility and professionalism in a fast‑paced environment with frequent interruptions

• Active state LPN license
• Minimum of 3 years of experience in long‑term care or skilled nursing
• Current BLS/BCLS certification
• Strong medication administration and documentation skills
• Ability to work independently and exercise sound clinical judgment
• Excellent communication and interpersonal skills
• Compassionate, patient‑centered approach to care

• Travelers must reside at least 50 miles from the work location
• Assignment may include up to three shift cancellations during the 13‑week term
