Job Summary:

As a Registered Nurse (RN), you will play a crucial role in providing compassionate and skilled nursing care to patients across d'Youville and St. Mary's. Your responsibilities will include assessing patient health needs, developing, and implementing nursing care plans, and evaluating patient responses to treatment. You will collaborate with interdisciplinary teams to ensure patient care and communicate effectively with patients, families, and healthcare providers.

Additionally, you will administer medications, perform medical procedures, and educate patients and their families on self-care promotion and disease prevention. • Coordinate the Fracture Liaison Service (FLS), assessing patient needs and guiding them through the FLS process including arranging labs, medications, and referrals.
• Collaborate with medical champions and clinical departments to establish clinical protocols for the FLS.
• Assess patient educational needs and assist in developing personalized care plans, offering support for smoking cessation, exercise, and dietary changes.
• Establish efficient systems for data retrieval and serve as a liaison between inpatient orthopedic care and outpatient clinics.
• Perform acute inpatient and office outpatient documentation using EMR systems.
• Serve as a vital link between patients and other healthcare providers, offering support and resources.
• Provide FLS presentations to community, external agencies, and internal staff.
• Maintain patient information databases and compile data for registries like the American Joint Replacement Registry.
• Monitor quality indicators, analyze patient outcomes, and implement improvements.
• Review charts to assess complication rates and provide post-analysis evaluation to the Director of Orthopedic Service Line.
• Ensure program standards meet certification guidelines and established care standards.
• Identify learning needs and pursue continuing education opportunities.
• Stay updated on advancements in the field and engage in community service.
• Prepare annual self-appraisals, maintain certifications, and demonstrate initiative in problem-solving.
• Promote the Mission, Vision, and Values of St. Mary's Health System/Covenant through effective communication and adherence to organizational standards.
• Complete assignments in a timely manner, resolve conflicts using appropriate communication channels, and utilize effective written communication skills.
• Demonstrates awareness of industry advancements and best practices.
• Maintains competency through self-evaluation and training programs.

• 3-5 years' experience of nursing experience preferred.
• 2-year associates or 4-year bachelor's degree in nursing.
• Experience in clinical office setting preferred.
• BCLS required.
