LPN - Nurse Navigator Licensed Practical Nurse - Orthopedics

Location:

Geisinger Gastroenterology & Endoscopy, Lewistown.

Shift: Days (Monday‑Friday 7:45a.m. – 4:15p.m.).

Schedule:

40hours weekly; 8‑hour shifts. Primary site:
Geisinger Lewistown Clinic, with 1‑2 days per week travel to Grays Woods.

Licensed Practical Nurse serving as a Patient Navigator, guiding incoming calls and facilitating patient entry into the program. The role involves patient education, continuity of care, and collaboration with the clinical team.

• Guide incoming patient calls and facilitate entrance of new patients into the program.
• Provide patient education and promote understanding of conditions while ensuring continuity of care.
• Deliver care in a team environment using clinical and didactic knowledge.
• Assist team members and provide clinical support to providers and patients for diagnosis and treatment.
• Conduct patient evaluations by telephone or document review and document needs.
• Coordinate and implement patients’ plans of care, including evaluation, treatment, and surveillance.
• Communicate with patients via mail, telephone, and in person regarding care information.
• Coordinate patient needs and activities such as support groups and educational series.
• Serve as point of contact for providers and scheduling services.
• Develop and implement patient screening tools and efficient clinic procedures.
• Investigate patient concerns including appointment access, communication, and billing.
• Maintain clinical supply inventory and perform equipment maintenance and inspections.
• Conduct medical record reviews and assist physicians with procedures.

This full‑time position consists of 8‑hour shifts Monday through Friday. It is primarily located at the Geisinger Lewistown Clinic with occasional travel to Grays Woods.

• Minimum of 2 years licensed practical nurse (LPN) work experience.
