The Hospital Liaison serves as the primary with hospital patient referral sources and, in many cases, is the initial point of contact for potential admissions to the skilled nursing facility (SNF). The Hospital Liaison is responsible for: assisting referral sources in identifying and assessing hospital patients with care needs that match the services provided by the SNF; educating and selling the potential patient and family regarding the SNFs’ services, admissions process, and treatment process;

and communicating with appropriate SNF staff to ensure appropriate admission of new patients.

Job Requirements:
  • Ensures that facilities achieve monthly budgeted census

  • Assists referral sources in identifying patients with care needs that match the services provided by the Hospital

  • Complete preliminary, standardized assessments on each new patient referral

  • Educate the potential patient and family regarding services provided, admissions process, treatment process and projected outcome

  • Communicate with appropriate nursing facility staff and ensure accommodations and staff are prepared for patient arrivals

  • Coordinate patient transfers and necessary transfers and admissions paperwork

Minimum Qualifications:
  • Must have 3 years of successful, high volume SNF admission/marketing experience in DFW

  • Ability to communicate with all levels of personnel

  • Must have excellent assessment skills
