Qualifications:

• Must be a DDS or DMD from an accredited school
• Successful completion of an advanced education program in endodontics accredited by the Commission on Dental Accreditation of the ADA required.
• Current, valid
  
  Kentucky license to practice dentistry(License must be ingoodstanding) or eligible for licensure
• Other certifications as required bystate toinclude-CPR, DEA,etc.
