Job Summary
The Bluegrass Care Clinic is seeking an individual to become an integral part of the multidisciplinary Infectious Disease care team to provide intensive prevention case management services to educate, promote healthy lifestyle choices and contribute to the Bluegrass Care Clinic's mission of providing quality medical services to people who use drugs.
ResponsibilitiesThe primary responsibilities of this position will include: educating patients on infectious disease transmission, high risk behaviors and treatment adherence, engaging clients in health discussions, developing behavior care plans and providing education and incentives to encourage and promote healthier lifestyle choices among patients.
Skills / Knowledge / Abilities- MS Windows, Excel, Access (or other database experience), Outlook
- Written/oral communication, planning/organizing, continuous improvement, medical knowledge, teaching skill, collaboration, attention to detail, basic statistics, quantitative analysis, and computer skills
Master's degree in Health Education, Social Work, Social Services, Public Health, Epidemiology, Criminal Justice, or Human Development. Bilingual in Spanish preferred.
ShiftMonday-Friday, Day Shift; 8:00am‑5:00pm (other days and hours as needed).
Physical RequirementsWalking with items up to 10 lbs.
