As a Bluegrass Care Navigators' Hospice Team member, you will be a part of a team providing care and support to patients and their families who no longer receive treatments to cure their terminal illness but instead choose care to improve their comfort and quality of life. Our service allows our team members to focus on one patient at a time providing care where the patient calls home.
While this often is a house or apartment, we also provide care at nursing homes, assisted living facilities, and hospitals. If a patient’s care needs are too complicated for home care or require a higher level of attention from nurses and other experts, we can provide care at one of our Hospice Care Centers.

Schedule: Monday-Friday 9:00am-7:00pm, with 1 day off per week
Location: Fayette County and Jessamine County
Position SummaryThe Hospice Admissions Nurse serves as the first point of clinical contact for patients and families entering hospice home care. This role is responsible for assessing patient eligibility, completing comprehensive nursing assessments, and initiating individualized care plans in accordance with hospice guidelines. The Hospice Admissions Nurse educates patients and families about hospice philosophy, services, and care processes while ensuring compassionate, patient-centered support during a critical transition.
The nurse collaborates with the interdisciplinary team, physicians, and referral sources to facilitate timely and seamless admissions, ensuring regulatory compliance and the highest standards of care. A Registered Nurse in this position will have compassion, strong communication skills, and the ability to provide emotional support during a sensitive time are essential to success in this role.
- Graduate of an accredited school of registered nursing.
- Currently licensed as a Registered Nurse through the Kentucky Board of Nursing or through a participating Compact State Board of Nursing.
- Minimum of one (1) year of general nursing experience preferred; new graduate nurses will be considered.
- Demonstrates strong clinical nursing skills and the ability to maintain professionalism during emotionally challenging situations.
- Provides compassionate, patient-centered care while serving diverse populations with respect and cultural sensitivity.
- Adheres to accepted professional, ethical, and legal standards of nursing practice.
- Demonstrates understanding of and commitment to hospice philosophy and the principles of death and dying.
- Maintains current CPR certification.
- Must be a licensed driver with an automobile in good working order, that is insured in accordance with state minimum requirements.
