Position Summary:
The Intake Registered Nurse (RN) is responsible for completing key components of the PACE intake and enrollment process. This role ensures that prospective participants receive timely, comprehensive nursing assessments and that the Interdisciplinary Team (IDT) is provided with accurate clinical information to support appropriate enrollment decisions. The Intake RN conducts home and clinic visits, evaluates clinical and functional needs, and supports a smooth, compliant transition into the PACE program.
Position Qualifications:
- Must be a licensed driver
- Graduate of an accredited school of registered nursing.
- Currently licensed as Registered Nurse through the Kentucky Board of Nursing or a participating Compact State Board.
- Minimum of one (1) year of nursing experience working with the frail and elderly.
- Experience in home health, geriatrics, primary care, hospice, palliative care, preferred.
- Food handling permit required; to be completed upon hire.
- Demonstrates strong nursing assessment skills, sound clinical judgement, and the ability to identify medical, functional, and psychosocial needs during intake and enrollment evaluations.
- Skilled in communicating effectively with participants, families, caregivers, referral sources, and interdisciplinary team members; able to cope with family emotional stress and respect diverse lifestyles.
- Demonstrates the ability to provide clear, timely, and accurate clinical documentation in the electronic health record
- Possesses and maintains current CPR certification.
- Have excellent observation skills, good nursing judgment and communication skills. Ability to provide good written documentation in a timely manner
- Self-directed with the ability to work independently while effectively collaborating within an interdisciplinary team environment.
- Flexible and cooperative in fulfilling role responsibilities and supporting program needs
