Nurse Practitioner

Responsibilities

• Provide comprehensive care to seniors following clinical standards.
• Collaborate daily with care team to ensure coordinated, high‑quality care.
• Manage referrals, hospital/SNF coordination, DME, and home health services.
• Document care with support from quality‑based coders.
• Participate in on‑call rotation and support growth plans at your center.
• Have experience providing value‑based care.

• Bachelor’s degree.
• Master’s in Science of Nursing / PA.
• Active, unrestricted Nurse Practitioner or PA license in the appropriate state.
• Passion for improving consumer experiences.
• Willingness to participate in limited on‑call coverage by phone.
• Ready to undergo TB screening as part of Tuberculosis (TB) screening program.
• Onsite location:
  Richmond Road clinic.

Scheduled weekly hours: 40. Base pay range: $123,800 - $170,300 per year. Eligible for a bonus incentive plan based on company and/or individual performance.
