As a Licensed Practical Nurse (LPN) at Lexington Premier Nursing and Rehab
, you will be responsible for assessing patient needs, providing direct nursing care, and collaborating with the healthcare team to ensure the best outcomes for our residents. You will work in a supportive and dynamic environment where your skills and dedication contribute significantly to the well‑being of our residents.
- Provide direct nursing care to residents, including administering medications, wound care, and treatments as prescribed by the physician.
- Monitor residents’ vital signs, document their progress, and report any significant changes to the Charge Nurse or Director of Nursing.
- Assist in the development and implementation of individualized care plans, ensuring they are tailored to meet each resident’s needs.
- Maintain clear and accurate documentation of patient care, and communicate effectively with residents, families, and the healthcare team.
- Adhere to all facility policies, procedures, and regulatory requirements, including infection control and safety protocols.
- Provide emotional support and education to residents and their families, helping them understand care plans and health conditions.
- Work collaboratively with other healthcare professionals, including RNs, CNAs, physicians, and therapists, to deliver coordinated and comprehensive care.
- Valid LPN license in Kentucky.
- Graduation from an accredited LPN program.
- Previous experience in a skilled nursing facility or similar healthcare setting preferred.
- Current CPR/BLS certification required. Additional certifications may be required based on state regulations.
- Excellent verbal and written communication skills. Ability to work collaboratively with a diverse team of healthcare professionals and build positive relationships with residents and their families.
- Ability to lift and move patients, perform physical assessments, and stand for extended periods.
- Ability to perform assessments.
- Administer medications.
- Provide wound care.
- Excellent communication and interpersonal skills.
