- Report to the Associate Medical Director of KY:
Dr. Cheryl Mcclain. - Work on site at Center Well Regency: 171 W Lowry Ln Suite 180 Lexington, KY 40503.
- Deliver comprehensive outpatient care to adult and senior patients.
- Maintain accurate medical records and documentation.
- Diagnose and manage moderately to complex medical conditions.
- Coordinate referrals and collaborate with specialists.
- Participate in clinical quality improvement programs.
- Work with interdisciplinary teams to ensure holistic care.
- Contribute to strategic programs and innovations in care delivery.
- Exercise independent clinical judgment in patient management.
- Master’s degree in nursing or completion of a PA program with board certification.
- Active, unrestricted NP or PA license in the state of practice.
- Willingness to participate in quarterly limited on‑call coverage by phone.
- Proficiency with electronic health records and digital documentation.
- 2+ years’ experience working as a NP or PA in an outpatient clinical setting.
- Specialty training in an outpatient clinical setting in Primary Care, Family Medicine, Internal Medicine, or Geriatrics.
- Experience working with senior populations or in value‑based care settings.
- Proficiency with electronic health records Athena Health and digital documentation.

Monday–Friday, 8:00 AM – 5:00 PM.
This role is considered patient facing and is part of Humana’s tuberculosis screening program. If selected for this role, you will be required to be screened for TB.

Pay Range$123,800 – $170,300 per year. This job is eligible for a bonus incentive plan based on company and/or individual performance.
