Job Summary
An Advanced Practice Provider (APP) is a licensed and board-certified nurse practitioner or physician’s assistant assigned to Longevity Health Plan members to execute the Institutional Special Needs Plan Model of Care. APPs oversee a panel of nursing home residents, conduct comprehensive assessments, develop individualized plans, attend care team meetings, and communicate with families and care teams so goals of care are understood.
APPs focus on treating in place, using clinical practice guidelines to guide treatment decisions, and document health status.
- Provide collaborative primary care and in‑home services to plan members.
- Execute model of care activities, including advanced care planning and treating in place.
- Facilitate and administer health plan clinical operations—prior authorization, Part A/B services, medication reviews, quality gap closure.
- Serve as trusted advisor and advocate for members, families, and nursing staff.
- Coordinate with PCPs and telehealth partners on care needs, goals, and plan changes.
- Promote Longevity Health Plan to members, families, facilities and stakeholders.
- Function autonomously, establishing relationships in the nursing home environment, balancing caseloads and meeting market goals.

Credentials & Coverage- State unrestricted licensed Advanced Practice Nurse or Physician’s Assistant with national board certification (AANP, ANCC, AACN, or NCCPA) as Adult/Family/Geriatric/Primary Care NP or PA.
- DEA license required.
- Active NPI and Medicare provider number; eligible to participate in Medicare program.
- Valid state driver’s license, good driving record, proof of automobile insurance with required liability limits (minimum $100,000/$300,000 bodily injury, $100,000 property damage, combined single limit $300,000).
Master of Science in Nursing as a Nurse Practitioner or Master’s Degree from a nationally accredited program. Minimum 6 months experience as NP or PA, preferably in nursing home environment. Must have some experience with geriatric/hospice or palliative care population or be a new graduate NP/PA supervised in the LHP New Graduate Fellowship. Computer/technology proficiency and strong communication skills required. Excellent clinical skills including assessment, diagnosis, treatment, prescribing for chronic illness.
Critical thinking, organization, and prioritization skills.
- 6+ months experience as NP or PA.
- Master of Science in Nursing or equivalent Master’s Degree.
- Experience in nursing home setting strongly desired.
- Computer/technology proficiency.
- Effective communication with physicians, members, families.
- Self‑directed but functions well in team.
- Excellent clinical skills in assessment, diagnosis, treatment, prescribing for chronic illness.
- Demonstrated critical thinking, organization, prioritization.
