As a Licensed Practical Nurse (LPN) at Lexington Premier Nursing and Rehab
, you will be responsible for assessing patient needs, providing direct nursing care, and collaborating with the healthcare team to ensure the best outcomes for our residents. You will work in a supportive and dynamic environment where your skills and dedication contribute significantly to the well‑being of our residents.
- Patient Care:
Provide direct nursing care to residents, including administering medications, wound care, and treatments as prescribed by the physician. - Monitoring and Assessment:
Monitor residents' vital signs, document their progress, and report any significant changes to the Charge Nurse or Director of Nursing. - Care Planning:
Assist in the development and implementation of individualized care plans, ensuring they are tailored to meet each resident's needs. - Communication:
Maintain clear and accurate documentation of patient care, communicate effectively with residents, families, and the healthcare team. - Compliance:
Adhere to all facility policies, procedures, and regulatory requirements, including infection control and safety protocols. - Support:
Provide emotional support and education to residents and their families, helping them understand care plans and health conditions. - Team
Collaboration:
Work collaboratively with other healthcare professionals, including RNs, CNAs, physicians, and therapists, to deliver coordinated and comprehensive care.
- Licensure:
Valid LPN license in Kentucky - Education:
Graduation from an accredited LPN program. - Experience:
Previous experience in a skilled nursing facility or similar healthcare setting preferred. - Certifications:
Current CPR/BLS certification required. Additional certifications may be required based on state regulations. - Communications:
Excellent verbal and written communication skills. Ability to work collaboratively with a diverse team of healthcare professionals and build positive relationships with residents and their families. - Physical Requirements:
Ability to lift and move patients, perform physical assessments, and stand for extended periods.
