Position Overview

- Licensed Practical Nurse with a current license in the state of employment.
- Minimum of one year of LPN experience in an acute care setting (two years preferred).
- Home‑care experience desired.
- Effective communication skills and a positive impression with patients, families, physicians, and co‑workers.

- Provide skilled nursing care in a patient’s home as ordered by the attending physician.
- Perform care following order sets and goals set by the RN.
- Observe and monitor patient conditions.
- Notify the RNCM of any changes in patient condition.
- Administer medication as prescribed by the physician.
- Counsel patients and families on disease/injury processes and management.
- Incorporate patients and families in the development of the plan of care.
- Help decrease rehospitalizations by teaching prevention and good decision making.
- Listen to patients and family members.
- Complete all clinical documentation per agency protocol and Medicare/Federal guidelines.
- Maintain an active LPN license in the state of employment.
- Communicate with scheduler regarding any changes outside normal schedule.
- Participate in all on‑call requirements and case conferences.
- Attend in‑service trainings and mandatory agency meetings.
- Maintain a professional appearance as a representative of the company.
- Understand and practice agency policies and procedures.
- Perform other duties as assigned.
- Drive to the patient’s primary location per scheduled visit.
- Attend assigned visit locations daily.
- Document all aspects of subsequent visits within 24hours to meet Medicare guidelines.
- Complete and submit required documentation within specified company requirements.
- Provide skilled care within the scope of practice for a Licensed Practical Nurse.
- Fulfill LPN visits in the assigned geographic location per patient need within federal and state specifications.
- Travel required.
- Reading, speaking, writing, and general communication in English.
- Computer/PDA usage.
- Hand/finger dexterity.
- Hearing and vision for close work.
- Walking, sitting, driving, talking on the phone.
- Bending, standing, lifting (up to 50lb with or without assistance).
- Stretching, reaching, climbing, stooping.
- Other physical activities as necessary.
