As a Bluegrass Care Navigators' Hospice Team member, you will be a part of a team providing care and support to patients and their families who no longer receive treatments to cure their terminal illness but instead choose care to improve their comfort and quality of life. Our service allows our team members to focus on one patient at a time providing care where the patient calls home.

While this often is a house or apartment, we also provide care at nursing homes, assisted living facilities, and hospitals. If a patient’s care needs are too complicated for home care or require a higher level of attention from nurses and other experts, we can provide care at one of our Hospice Care Centers.

Schedule: 4X10-hour shifts, 8:00am-7:00pm EST, every other weekend. (Example of a schedule:
Work Sunday, off Monday/Tuesday, work Wednesday-Friday, off Saturday and Sunday, work Monday-Wednesday, off Thursday-Friday, work Saturday.)

Position Summary:
The Support Center LPNis a member of the interdisciplinary team who work together tomeet the physical, medical, psychosocial,emotional,and spiritual needs of BCN’s patients and families facing serious illness.

The Support Center LPNperformstelehealthtriage functions including but not limited to receiving calls;providing phone support;providing interventions utilizing BCN policies,procedures,and protocols;dispatching on-call nursesas needed. The Support Center LPN completes documentationutilizingthe EMR, point of service.

This role will work under the guidance of the Support Center RN Supervisor.

Position

Qualifications:

• Is a graduate of an accredited practical nurse or vocational nursing program.
• Has at leasttwo(2) years of nursing experience.
  
  Community Health/Hospice or long-term care experience preferred.
  
  Telehealthexperiencealsopreferred.
• Is currently licensed as an LPN through the State Board of Nursing.
• Has excellent assessment, nursing judgment, and communication skills.
• Ableto cope withheightenedfamily emotional stress, andtolerantof individual lifestyles.
• Complies with accepted professional standards and principles.
• Complies withstrictadherence to organizational policies,procedures,and protocols.
• Basic computer skillsare required. Advanced computer skillsare preferred.
• Is self-directed with the ability to work with little supervision.
• Is flexible and cooperative in fulfilling role obligation andhas the ability towork with an interdisciplinary team.
• Team member swill be expected tocompli with Bluegrass Care Navigators(BCN)
  Employee Health, infectious disease, and vaccination policies.
• Must be a licensed driver with an automobile in good working order, that is insured in accordance with state minimum requirements.
