Bluegrass Care Navigators is seeking a Hospice Team member to join a compassionate care team delivering comfort-focused services in patients' homes, nursing facilities, or care centers.
The role covers managing referrals, coordinating admissions, maintaining patient records in the EMR, and providing telehealth triage and phone support according to BCN policies.
This PRN position offers flexible 5-hour shifts, up to 10 hours per week with some weekend coverage and no more than 40 hours a month.
