Registered Nurse or LPN - Support Center - PRN

As a Bluegrass Care Navigators' Hospice Team member, you will be a part of a team providing care and support to patients and their families who no longer receive treatments to cure their terminal illness but instead choose care to improve their comfort and quality of life. Our service allows our team members to focus on one patient at a time providing care where the patient calls home.

While this often is a house or apartment, we also provide care at nursing homes, assisted living facilities, and hospitals. If a patient’s care needs are too complicated for home care or require a higher level of attention from nurses and other experts, we can provide care at one of our Hospice Care Centers.

Schedule:

PRN- coverage as needed (5-hour shifts - 10 hours per week). Weekdays will vary. Some weekend coverage as well. Likely no more than 40 hours a month.

Position Summary An employee in this position will manage referrals toThe Organization, coordinating the process through the admission of the patient while managing referral databases.

This employee will also be performing telehealth triage functions including but not limited to receiving calls; providing phone support; providing interventions utilizing BCN policies, procedures, and protocols; dispatching on-call nurses as needed, completing documentation utilizing the EMR, point of service and making appropriate notifications to primary team regarding any changes in LOC.
