  • $300 Sign‑on bonus after 90 days of service (FT or PT, 24 hours per week)
  • $300 Referral Bonus for employee referred hires who complete 90 days and work FT or PT hours
General Function

The position involves providing support to individuals with disabilities, empowering them to lead self‑directed lives and helping them develop and maintain skills that enhance the quality of their lives. The mission is to ensure that supported persons are happy, healthy and safe in an environment where they can enjoy life to the fullest extent possible.

Essential Functions
  • Support the person with personal care and daily routines, and help achieve outcomes from the Individual Support Plan.
  • Understand and practice Support Solutions philosophy and values, treating everyone with dignity and connecting the person to preferred activities and experiences.
  • Help the person become integrated with the community by providing person‑centered supports to pursue their goals and relationships.
  • Ensure safe transport and accompany the person as needed to events, job sites, and medical appointments using company vehicles, personal vehicles, or public transportation.
  • Ensure proper use of the person’s and agency resources, including personal funds, vehicles, buildings, utilities, telephones, money, equipment, food, and supplies.
  • Attend all training meetings assigned by Support Solutions.
  • Cook and provide meals based on scheduled menus.
  • Perform daily activities or teach the person to conduct activities of daily living toward independence.
  • Consistently document and track improvement toward meeting outcomes.
  • Use the electronic records system, including completing daily shift notes as required by funding entities.
  • Ensure customer satisfaction by following Customer Service Standards when interacting with persons supported, families, conservators, vendors, ISCs, Case Managers, and others.
  • Oversee storage and administration of medications.
  • Assist the person with personal grooming, encourage independence whenever possible.
  • Assist the person with housekeeping tasks such as cleaning and laundry as desired.
  • Protect confidentiality of the person supported.

Minimum Requirements
  • Must be 18 years of age and able to read and write effectively.
  • High school diploma or GED.
  • Valid driver’s license and acceptable driving record throughout employment.
  • Meet state and local requirements for criminal background check, sexual offender check, abuse registry, drug and felony offender listings.
  • Meet all training requirements and keep training current.
Knowledge, Skills & Abilities
  • Effective verbal and written communication skills.
  • Effective organizational skills, able to complete heavy workloads within established time frames and perform with frequent interruptions.
  • Effective interpersonal skills, able to establish and maintain cooperative relationships with others and interact appropriately in various contexts.
  • Practice confidentiality in all matters pertaining to clients and employees.
  • Perform a variety of duties, often changing tasks frequently due to varying technology, techniques, environmental factors, physical demands, or work situations.
Medical Requirements
  • Provide medical records as required by governing entities and Support Solutions policy (e.g., TB skin test, HEP B vaccination, flu vaccination).
Physical Requirements
  • Body Positioning: Stand and walk for prolonged periods; sit for short or long periods; stoop, kneel, crouch and reach with hands and arms; drive/travel to transport the supported person.
  • Body Movements: Perform CPR compressions, abdominal thrusts, CPI techniques; pursue on foot a person to prevent injury; bend, reach, crouch, stoop, and climb when needed.
