ATTEND/Attendance Support Case Manager

Location:

Warren County ESC Main Office Lebanon

The WCESC/Coordinated Care Attendance Department is seeking a full time person to work with Warren Co. Juvenile Court and Carlisle Schools. This position will work with children/youth up to age 21. All youth served are at risk of further system involvement due to school attendance and potentially other concerns as well. The main function of the ATTEND/Attendance Support Case Manager will be to connect children and youth to services as referred by Warren Co.

Juvenile Court. In addition this position will monitor school attendance for Carlisle City Schools. This position will report to the Supervisor of Community Programs and Supports.

  • Assist families in strength based interventions
  • Assist youth in preventing further contact with the court and or other services
  • Provide services in the home and or community
  • Assist in monitoring student attendance
  • Assist in monitoring outcomes
  • Interface with school districts and county agencies to assist with the needs of youth and families
  • Coordination of resources such as food, clothing, school supplies and holiday programs as needed
  • Assist in the prevention of issues that hinder the educational development of a youth
  • Conduct CANS assessments (child, adolescent, strengths and Needs)
  • Assist in the mission of the Attendance Department

Candidates must have a valid state driver's license, bachelor's degree in social services or related field, experience working with children/youth with behavior problems, ability to track outcomes through electronic health record system and have excellent communication skills. Ohio license as a LSW or LPC is preferred but not mandatory.
