As a Bluegrass PACE Care team member, you will be a part of a program providing all-inclusive healthcare that keeps Kentuckians aged 55+, who may otherwise need nursing home care, living safely  PACE experts work collaboratively with an interdisciplinary team to decide what’s best for each PACE participant, bringing all aspects of medical and personal care, plus social and support services.

Position Summary:

ThePACESocial Worker provides and coordinates psychosocial services for participants and their families in thePACE Center, home, hospital, nursing home, or assisted living facility (ALF). The role also provides community-based case management services for program participants.

Position

Qualifications:

• Is a graduate of amaster’sprogram in Social Work accredited by the Council of Social Work Education;and is licensed by the state of Kentucky as a Certified Social Worker (CSW) or a Licensed Clinical Social Worker (LCSW).
• A minimum of twoyears’social service experience working in a case management capacity.
• Must have at least one year of experience working with the frail and elderly.
• Strongdemonstratedcapabilities in networking with community resources, and strong knowledge of such resources.
• Strong communication skills, including face-to-face, telephone, and computer skills.
• Strength in attention to detail and organization.
• Must be able to interact successfully with participants, families,and other agencies to present a positive and professional image.
• Knowledge and basic understanding of the interpersonaldynamicsoperative within the working relationships of an interdisciplinary healthcare team.
• Ability to manage multiple priorities simultaneously and effectively handle the emotional stress of the workload.
• Ability to workunsupervisedexercising a high degree of discretion & independent judgment.
• Ability to physically access any home, hospital, nursing home, and ALF (i.e., navigate stairs, narrow stairs, narrow spaces, lack of air conditioning, etc.).
