Cardinal Hill Rehabilitation Hospital, part of Encompass Health, seeks a Senior Case Manager to coordinate complex rehabilitation care for adult inpatients. In this role, you will manage a caseload from admission through discharge, developing individualized care plans, facilitating family conferences, and ensuring safe, timely transitions. You will collaborate daily with physicians, rehab nurses, therapists, and social workers to align goals, remove barriers, and secure community resources.
The Senior Case Manager also supports utilization review, documentation accuracy, and performance improvement initiatives to drive quality outcomes and patient satisfaction.
Responsibilities
- Manage an inpatient rehabilitation caseload from admission through discharge
- Develop and update individualized care and discharge plans
- Coordinate interdisciplinary team meetings and family conferences
- Conduct psychosocial assessments and identify barriers to recovery
- Arrange post-acute services, equipment, and community resources
- Perform utilization review and support appropriate length of stay
- Ensure accurate, timely documentation and regulatory compliance
- Educate patients and families on care plans, expectations, and resources
- Collaborate with payers and external providers to support safe transitions
- Participate in performance improvement and outcome initiatives
