Bluegrass Care Navigators in Lexington, KY, is seeking a compassionate Social Worker for the PACE program. You will coordinate psychosocial services for participants and families, work with an interdisciplinary team, and help keep seniors living safely at home.
Schedule:
Monday–Friday; provides community-based case management in the PACE Center, home, hospital, or ALF. Requires MSW CSWE-accredited and KY CSW/LCSW; two years in social work; strong networking and communication skills.
