A growing Home Care business is seeking a Case Manager to join our team - overseeing quality of care for assigned clients and supporting our clinical efforts.
Responsibilities may include supervising/training caregivers, advocating internally/in the community for our clients, conducting assessments, taking lead calls, addressing client's clinical needs, authoring and monitoring care plans, on-boarding new clients, and more. We are looking for a clinician who can convey their high clinical standards to deliver best-in-class in-home care.

Experience:
- · Working with elders and/or disabled populations: 1-2 year (Preferred)
- Clinical Healthcare experience with appropriate license/certification
- Reliable transportation
- Clean background check/Clean driving record
- Some supervisory experience
- Therapists/social workers have been successful in this role
