As an RN Case Manager, here's a big-picture view of what you'll do:
* Oversee the delivery of clinical and caregiver services, ensuring the highest quality of patient/client care in accordance with home health laws and regulatory standards
* Working closely with Director of Health Care Services and Administrator to do OASIS admissions, Recerts, Resumptions, DCs, case management and Coordinate with a team of LPNs, CNAs, aides and to ensure patient goals are met
* Follow the Home Health orders from PCPs working closely with their office
* Ensure medical supplies/equipment are delivered and staff are trained in care procedures
* Assess patient/client needs, resolve issues, and ensure a high level of satisfaction
* Effectively manage financial resources related to the delivery of quality patient/client care
A few must-haves for a Case Manager RN:
* Graduate of an accredited nursing program and active RN license in Massachusetts or BSN
* Minimum of 1 year of nursing experience in home healthcare/ OASIS admissions, recerts, resumptions and DC's
* At least 1 year of supervisory and caregiver management experience
* CPR certification
* Extensive knowledge of state and federal home health laws and regulatory standards
* Excellent management, organizational, problem-solving, communication and interpersonal skills
