The CNA is accountable for the delivery of quality home care services to the program's participants in conjunction with physician directions, company and industry standards, and insurance guidelines as assigned by a registered nurse. The CNA is supervised by a licensed nurse and works under the supervision of the Home Care RN and Health Care Coordinator (RN), with indirect supervision from the Director of Activities.

The CNA provides assistance with activities of daily living, restorative and supportive care, assists with activity programming, monitors health status, and functions as a member of the Interdisciplinary Team.

Essential Duties and Responsibilities:

Provide home care services according to participant care plans and nursing instructions under the supervision of the Home Care Coordinator RN or Clinical Supervisor.

Observe, document, and report changes in participants' physical, emotional, and functional status to the appropriate clinical staff.

Assist participants with activities of daily living, including bathing, dressing, grooming, toileting, eating, and mobility support.

Monitor and assist participants during meals and encourage proper nutrition and hydration.

Create and participate in group and one-on-one activities in accordance with the daily activity calendar.

Assist in maintaining a clean, safe, and orderly day room, kitchen, and personal care areas.

Accompany participants to medical and other appointments as needed.

Assist with transportation services, including loading and unloading participants and serving as an escort when necessary.

Participate in Interdisciplinary Team meetings and carry out participants' plans of care.

Maintain participant records and complete required Every Age PACE documentation and reporting.

Follow all Every Age PACE policies, OSHA guidelines, infection control procedures, and confidentiality requirements.

Participate in required staff meetings, in-service education, and quality improvement initiatives.

Perform other duties as assigned by supervisor.

High school diploma or equivalent required.

Current Cardio Pulmonary Resuscitation (CPR) certification required.

Minimal

Experience:

A minimum of one year of experience working with the frail or elderly population required.
